The Aging Society and Thai Elderly Welfare: Perspectives from Medical Social Work Practices – A Case Study of Cardio-Pulmonary Patients and the Application of the Social Telecare Platform

Abstract

This academic article aims to analyze the role and practice of Medical Social Workers (MSWs) in providing holistic social welfare services to older adults in Thailand's rapidly aging society with a specific focus on patients with complex cardiopulmonary conditions following the guidelines of the Chest Disease Institute. The study utilizes a case study of Mr. Somchai (pseudonym),  male who presented with acute chest pain and left-side numbness indicative of Acute Coronary Syndrome and potential cerebral complications, coupled with significant social issues such as caregiver burden and economic vulnerability within his family.

 

The analysis highlights the critical role of the MSW in conducting thorough social assessment to link essential welfare services (e.g., financial aid, home modification, long-term care) and in coordinating between the medical team and community networks. Crucially, the article proposes the application of the Social Telecare Platform as a key innovation to reinforce continuous post-discharge care. Expected outcomes include reducing the readmission rate, alleviating caregiver stress, and enhancing systematic access to social services remotely. This approach aims to sustainably improve the quality of life and social security for older adults with complex illnesses in Thailand.
