From Silence to Song: Integrating Music Therapy and Social Work Support for the Holistic Care of Terminally Ill Patients

Abstract

This analytical and integrative academic article aims to (1) explain the mechanisms and theoretical foundations of music therapy, (2) expand the role of social workers in providing psychosocial support, (3) propose practical approaches for integrating music therapy with social work practice to enhance the quality of life of end-of-life patients, and (4) analyze its application in the Thai context through a case example. End-of-life patients often experience profound silence in verbal communication and emotional expression due to physical pain, anxiety, and psychological preparation for death. The findings from the analysis and synthesis indicate that music therapy can alleviate pain, reduce anxiety, and open pathways for emotional and spiritual communication. Social workers play a crucial role in assessing psychosocial needs, providing counseling, and facilitating collaboration among families and interdisciplinary care teams, thereby ensuring that music therapy is aligned with patients’ life contexts and personal values. Such integration contributes to improved quality of life, enhanced peace, acceptance, and strengthened relationships during the final stage of life. In the Thai context, effective application requires sensitivity to cultural beliefs, values, and musical preferences. The case example demonstrates that collaboration between music therapy and psychosocial support can transform silence into a meaningful space of connection, love, and a dignified and peaceful dying process.
